Early Alert System

The Process

Refer eligible students to the Early Alert System through the use of the simple online form below. Please include as much detail as possible. All information will be handled confidentially and shared only with those individuals receiving a referral. Upon submitting the form, the referring faculty or staff member will see an immediate, automatic web response to confirm their submission. The submitted information is sent via e-mail to the Office of Student Success. Students referred to the Early Alert program are contacted initially by phone and/or e-mail by the Office of Student Success. The Office of Student Success then works to schedule an appointment with the student to discuss the concerns expressed by the reporter (typically faculty or staff) and to develop an academic success plan. As part of that plan, the Office of Student Success may refer students to other campus offices as appropriate. Ultimately, it is the student's responsibility to follow up and take advantage of the resources and referrals made available to them.

Submit an Alert

Please use this form to report any academic difficulty or any behaviors of concern involving the Notre Dame of Maryland University students, whether these behaviors occur inside or outside of the classroom setting. Reportable behaviors include a student who you feel may be struggling physically, psychologically, academically, or any other behaviors of concern that you would like to see addressed or would like to discuss. When completing this form, please include your name, and contact information. The information given remains private, meaning it will only be shared with University professionals who have a required need to know.
