Child Release Authorization

Please provide the contact information for individuals who are permitted to pick up your child from camp. If you are coordinating another party to pick up your camper one day that not listed below, written permission to the camp director.

Photo/Model Release Form

Throughout the day, STEM camp  staff often takes photographs and sometimes videos of our campers as they embark on various adventures throughout the day.

The photographs, showing the campers of the STEM camp interacting with each other, STEM staff and NDMU student workers may be used to promote STEM camp in the future.

We are asking for your permission to use those images on the STEM website which is located online, the Notre Dame of Maryland University website, STEM social media accounts, or in any brochures and flyers.

Only photos of campers whose parent/guardian completes and acknowledges this release form will be used. Thank you very much for your assistance in promoting the NDMU STEM camp.

Cancellation and Refund Policy

The NDMU STEM camp fee is $500 for a one-week session. Non-refundable deposit of $100 is due at the time of online registration (if it is before April 1, only $50 is due). This will be applied to the camp fee due for the session.

The remainder of the camp fee is due no later than one week prior to the beginning of camp. There will be no refund of the camp fee after the first day.

COVID-19 Waiver for Summer Programming

Notre Dame of Maryland University (“NDMU” or “the University”) adheres to federal, State, and Baltimore City COVID-19 related laws, regulations, and health guidelines and if for any reason the University makes the decision to not allow in-person summer programming for 2021, payment will be fully refunded or may be used for virtual camp registration.

Declaration COVID exposure:

I make the following statements with the safety and health of my camper as well as the Camp Notre Dame community in mind:

  • If my camper(s) or anyone in our immediate household is experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, muscle pain, headache, sore throat, or sudden loss of taste or smell, my camper(s) will not come to camp until a negative COVID-19 is received.
  • If my camper(s) or anyone in our immediate household has come in close contact with a person with a suspected or confirmed case of COVID-19 we have observed and completed quarantine in accordance with CDC quarantine guidance.
  • My camper(s) or anyone in our immediate household has not been diagnosed with COVID-19 or if they have we have been released from isolation in accordance with CDC isolation guidance.
  • My camper(s) and our immediate household are following all CDC-recommended COVID-19 guidelines as completely as possible in order to limit exposure to COVID-19 and the spread of COVID-19. *:

DECLARATION — RECENT TRAVEL

  • My camper(s) or anyone in our immediate household have not traveled outside the United States in the last 14 days.
  • My camper(s) or anyone in our immediate household have not traveled out of the State of Maryland (except for commuting purposes) in the last 14 days; or if they have traveled outside the State of Maryland, they have received a negative COVID-19 test.

NOTRE DAME OF MARYLAND UNIVERSITY CORONAVIRUS/COVID-19 WARNING, ASSUMPTION OF RISK, AND WAIVER

I hereby acknowledge that the coronavirus COVID-19 is an extremely contagious virus that spreads easily through person-to-person contact. I acknowledge that by my camper(s) attending NDMU summer camp or accessing NDMU’s facilities, it could increase our risk of contracting COVID-19. I understand and agree that neither NDMU nor SAGE Dining Services in any way warrants that COVID-19 infection will not occur through my camper(s) attendance at the above event or by accessing NDMU’s facilities. I hereby voluntarily seek to attend the above event and acknowledge that my camper(s) actions may increase my risk of exposure to the coronavirus COVID-19. I accept the risk and release and agree to hold harmless NDMU and/or SAGE Dining, the employees, directors, officers, and agents of either of them, and the heirs, successors, and assigns of any of them, from any and all claims that may arise from or relate to my camper(s) attendance at the above event or my camper(s) use of NDMU’s facilities, including any claims concerning exposure to the coronavirus COVID-19 and any resulting harm or injury, including permanent disability and death. I acknowledge and agree that during my camper(s)  attendance at the above NDMU summer camp, camper(s)  will comply with all procedures designed to reduce the spread of COVID-19.*:

INFORMATION PRIVACY

I agree that information submitted via this form may be shared with any state agencies, as needed, for the purpose of Covid-19 related contact tracing, should it become necessary. I also agree that information submitted via this form will be stored temporarily by NDMU, until it is no longer required as part of our ongoing Covid-19 health protocols.*:

STEM Behavior Policy

Notre Dame of Maryland University reserved the right to dismiss any camper, without refund, for behavior deemed disruptive to the effective operation of the camp.
